　　首先，要在假條的上方寫明假條是遞給誰的

　　To：假條是遞給誰的

　　From：請假人

　　Date：寫假條的日期(注意可不是請假的日期，不要弄混了哦!)

　　例如：

　　其次，在假條的第一段，要開門見山但是有禮貌地提出請假，要寫清你要請假的.日期。
